CONFIDENTIALITY
Hines Family Counseling & Coaching and its providers will hold any information revealed during counseling/ coaching in the strictest confidence. Information will not be revealed to any other person. The law does require certain limitations to confidentiality (see Limits to Confidentiality below). In situations when one member of the family is seen in the absence of others, the pastoral counselor/coach will encourage the individual to disclose to others in a safe environment. In the case of teens, parents have legal access to the information. We strongly encourage parents to weigh the harm to the teen before requesting the information.
LIMITS TO CONFIDENTIALITY:
As mentioned above, there are certain limitations to confidentiality. The law requires Pastoral/Christian counselors to reveal confidential information in certain situations.
LIMITATIONS INCLUDE:
• Any immediate threat of harm to yourself, the threat of you harming others,
• Any physical abuse, neglect, or sexual abuse of children or vulnerable adults.
• Any sexual exploitation by a health care professional.
• If State or Federal law authorizes the release of your records.
CANCELED, MISSED, OR LATE APPOINTMENTS
Hines Family Counseling requires a 24-hour cancellation notice for any scheduled appointment unless there is a verifiable emergency. Appointments canceled less than 24 hours prior to the appointment will be charged one session. Missing a scheduled appointment will result in being charged the full agreed upon fee that is due before the next appointment. Arriving late to a scheduled appointment will result in being charged the full agreed upon fee that is due before the next appointment. After two canceled, missed, or late appointments, Hines Family Counseling reserves the right to terminate services.
Hines Family Counseling understands that life can change quite frequently and with little notice. We know that our clients are adjusting to life's changes. Just call at us (918) 289-2033 or canceled online booking service (no emails) 24 hours prior to your scheduled appointment so that we may adjust as well.
This agreement for pastoral counseling service between Hines Family Counseling and client(s) shall govern all professional relations between the parties. It is agreed that any disputes or modifications of agreement shall be negotiated directly between the parties; if negotiations are not satisfactory, then the parties agree to mediate any differences with a mutually acceptable third party mediator.
The Pastoral Counselor is Johnathan Hines. He is a licensed minister, Licensed Christian Clinical Counselor through National Christian Counseling Association #11172, and Pastoral Counselor, not a state licensed therapist.
